PRODUCT DETAILS

Composition

  • Cefpodoxime proxetil (oral suspension)
    • Usually 40 mg/5 mL or 50 mg/5 mL (after reconstitution depending on product strength)

Category / Class

  • Third-generation cephalosporin antibiotic
  • Beta-lactam antimicrobial

 

Use / Indications

Used in bacterial infections, especially in children:

  • Acute otitis media (ear infections)
  • Tonsillitis / streptococcal sore throat
  • Sinusitis
  • Lower respiratory tract infections (bronchitis, pneumonia)
  • Some skin infections (susceptible organisms)

 

Mechanism of Action

  • Inhibits bacterial cell wall synthesis
  • Leads to bacterial cell death (bactericidal effect)

 

Direction of Use

  • Usually given twice daily (every 12 hours)
  • Dose based on body weight (commonly 8–10 mg/kg/day divided into 2 doses)
  • Must be reconstituted with water and shaken well before use
  • Best taken with meals to improve absorption and reduce stomach upset

Important handling:

  • Use measuring spoon/syringe (not household spoon)
  • Refrigerate after reconstitution (commonly 2–8°C)
  • Discard after about 10 days

 

Common Side Effects

  • Diarrhea
  • Nausea or vomiting
  • Abdominal pain
  • Headache
  • Rash or itching
  • Dizziness or vertigo

 

Serious Side Effects

Seek medical help if:

  • Severe allergic reaction (swelling, difficulty breathing)
  • Severe or bloody diarrhea (possible C. difficile infection)
  • Severe skin reactions (blistering, peeling)
  • Unusual bruising or infection signs (blood disorders)
  • Seizures (rare, usually overdose or kidney issues)

 

Warnings

Do NOT use in:

  • Allergy to cephalosporins (e.g., cefpodoxime)
  • Severe hypersensitivity to beta-lactam antibiotics

Important warnings:

  • Can cause allergic cross-reaction in penicillin-sensitive patients
  • Overuse may lead to antibiotic resistance
  • Dose adjustment may be needed in kidney disease
  • Diarrhea during or after treatment should be reported

 

Drug Interactions

  • Antacids (reduce absorption)
  • H2 blockers / PPIs (may reduce effect)
  • Probenecid (increases drug levels)
  • Warfarin (may increase bleeding risk)
  • Other nephrotoxic drugs (caution in kidney disease)

 

Cautions

  • Always complete full course even if improved
  • Take at same times daily (12-hour spacing)
  • May cause dizziness → avoid driving if affected
  • Monitor for fungal infections with prolonged use
  • Use cautiously in patients with:
    • Kidney impairment
    • History of allergy to penicillins/cephalosporins
