Composition

  • Nepafenac 0.1% ophthalmic suspension
    (1 mg/mL nepafenac)

Use / Indications

Used for:

  • Prevention and treatment of eye pain and inflammation after cataract surgery
  • Reduction of postoperative swelling
  • Reduction of risk of macular oedema after cataract surgery in diabetic patients

 

Mechanism of Action

Nepafenac is converted in the eye to amfenac, which inhibits cyclooxygenase (COX) enzymes and reduces prostaglandin production responsible for inflammation and pain.

Direction of Use

Typical dosage:

  • 1 drop into affected eye(s) 3 times daily
  • Usually started 1 day before cataract surgery
  • Continued on surgery day and for about 2 weeks afterward

 

Instructions:

  • Shake bottle well before use
  • Wash hands before application
  • Do not touch dropper tip
  • Wait at least 5 minutes between different eye medications
  • Remove contact lenses before use and reinsert after at least 15 minutes

 

Common Side Effects

  • Blurred vision
  • Eye redness
  • Eye irritation or pain
  • Watery eyes
  • Dry or sticky sensation
  • Feeling of something in the eye
  • Light sensitivity
  • Headache
  • Nausea

 

Serious Side Effects

Seek urgent medical attention if patient develops:

  • Severe eye pain
  • Vision changes
  • Eyelid swelling
  • Corneal damage or keratitis
  • Bleeding in the eye
  • Severe allergic reaction

 

Warnings

Do NOT use in:

  • Allergy to nepafenac or other NSAIDs
  • Patients with aspirin/NSAID hypersensitivity
  • Patients with active corneal problems unless supervised

Important warnings:

  • May delay wound healing
  • Prolonged use may increase risk of corneal complications
  • Increased risk of bleeding during eye surgery
  • Use cautiously in diabetes, dry eye syndrome, arthritis, or bleeding disorders

 

Drug Interactions

Use cautiously with:

  • Steroid eye drops
  • Other NSAID eye medications
  • Anticoagulants or antiplatelet drugs
  • Medicines that increase bleeding risk

 

Cautions

  • Temporary blurred vision may impair driving
  • Avoid wearing contact lenses during treatment
  • Long-term use may worsen dry eyes
  • Do not use longer than prescribed after surgery
  • Store properly and discard bottle about 4 weeks after opening
