Composition

  • Silver sulfadiazine 1% w/w
    (10 mg silver sulfadiazine per gram of cream) 

Category / Class

  • Topical antimicrobial / antibacterial cream
  • Sulfonamide derivative burn medication

 

Use / Indications

Used for:

  • Prevention and treatment of burn wound infections
  • Second-degree burns
  • Third-degree burns
  • Infected skin ulcers or wounds (in some cases)
  • Skin graft donor sites

 

Mechanism of Action

  • Silver ions damage bacterial cell membranes and enzymes.
  • Sulfadiazine inhibits bacterial folic acid synthesis.

Together, they provide broad-spectrum antimicrobial activity against many bacteria and some fungi.

Direction of Use

Typical application:

  • Apply a thin layer (about 1–2 mm thick) to cleaned burn/wound area
  • Usually once or twice daily

Instructions:

  • Clean and debride wound before application
  • Use sterile gloves or applicator if possible
  • Cover entire affected area continuously
  • Dressing may or may not be used depending on clinician advice
  • Reapply after bathing or wound cleansing  

 

Common Side Effects

  • Burning sensation
  • Itching
  • Rash
  • Skin discoloration
  • Pain at application site
  • Temporary leukopenia (low white blood cells) in some patients

Serious Side Effects

Seek urgent medical attention if patient develops:

  • Severe allergic reaction
  • Extensive rash or skin peeling
  • Fever/sore throat (blood disorders)
  • Liver problems
  • Kidney problems
  • Stevens-Johnson syndrome/toxic epidermal necrolysis (rare)
  • Severe skin discoloration

 

Warnings

Do NOT use in:

  • Sulfonamide allergy
  • Premature infants
  • Infants under 2 months
  • Near-term pregnancy
  • Severe G6PD deficiency without supervision

Important warnings:

  • May delay wound healing in some burns
  • Long-term extensive use may cause systemic absorption
  • Monitor blood counts in large burn areas
  • Avoid contact with eyes

 

Drug Interactions

Use cautiously with:

  • Other topical antimicrobials
  • Enzymatic debriding agents (may become inactive)
  • Sulfonamide-containing medications
  • Drugs affecting bone marrow

 

Cautions

  • Use cautiously in:
    • Kidney disease
    • Liver disease
    • G6PD deficiency
  • Avoid unnecessary prolonged use
  • Large surface area treatment may increase systemic effects
  • Keep treated area clean to prevent secondary infection
